Company Profile

Priority Senior Care Management, Inc. (PSCM) is a consulting team of Nurses, Social Workers, and Care Advisors who through years of experience in Home Health Care and Long Term Care, are well versed in the issues and resources related to long term care. Each Care Manager/Senior Care Advisor is well versed in the related issues and available resources.

We are also intimately familiar with all of the community resources available to seniors in Massachusetts. Priority Senior Care Management, Inc., provides a comprehensive and compassionate approach to patient care.

PSCM clinicians complete a comprehensive assessment in order to help family determine the best course of action whether it is home care with agency services and care management, or placement in the most appropriate facility. This assessment includes the social, psychological, medical, legal, environmental, and financial aspects of a person’s life, which gives the family and PSCM a very complete and detailed Plan of Care.

PSCM strives to work with the client and family to establish and implement goals for the client and family which are realistic and achievable. Clients and family are encouraged to be active versus passive members of the team and participate in the development of their individualized plan of care.

PSCM Geriatric Care Managers will assist in Coordinating resources, from healthcare providers as well as the community, and will Monitor the client’s progress and the delivery of services. Clients and their families can be assured of the quality of PSCM services as the PSCM Geriatric Care Managers, follow the Geriatric Care Management Standards as defined by the National Association of Professional Geriatric Care Managers.

Carolyn McCarthy is a Registered Nurse with over 20 years experience in community and home based services. In addition to a Masters Degree in Public Administration/Health Administration, Carolyn is a Certified Geriatric Care Manager through the National Academy of Certified Care Managers (NACCM)
