Geriatric Care Management Frequently Asked Questions

Q:  I have seen a big change in my mothers or fathers ability to take care of themselves. I'm not sure how serious the situation is but I know they need help. Can you help me?
A:  Yes. A Priority Senior Care, care manager first conducts an assessment of your Mom or Dad in their home. This includes a home safety review. The care manager identifies any problems, identifies community resources to meet their needs and develops a plan of care.

If you wish, your care manager can provide ongoing care management. Ongoing care management can include implementing the plan of care, coordinating services and monitoring the situation to see that any emerging needs are addressed. A typical case takes 4 - 10 hours of a care manager's time to complete the assessment and plan of care. The assessment and development of the plan of care is billed at a one time. Implementation and monitoring of the plan of care are billed at an hourly rate.

Q:  I know what type of care my loved one needs, but I need help finding a specific provider in my area. I don't have time to do research or make phone calls.  Can you help me?
A:  We can provide this type of consultation service by phone or in person. The care manager will provide you with carefully researched options, screened according to availability and your specific needs and preferences. Typically, this service will take 2-4 hours of a care manager's time, billed at an hourly rate.

Q:  Why won't you just give me a list of skilled nursing facilities, assisted living facilities, or home care agencies in my area?
A:  If we just gave you a list, you'd still have much work ahead of you. Instead, we develop a list of customized options that have been specifically screened according to your loved one's needs, resources and eligibility requirements. Our years of experience with Long Term Care have provided us with additional insight which would be helpful to you with this very important decision.

Q:  Not sure I can afford to pay for this. I can do my own research and make phone calls. How will a care manager help me?
A:  Our care managers have years of experience providing and researching care in the local community. For just a few hours of a care manager's time, you will have access to a wealth of knowledge and expertise that can benefit your loved one and make you feel confident you are making an informed decision. Your time is also valuable and you may be able to use this time to take care of yourself. It is easy to forget about caring for yourself during times of stress.

Q:  I work full time and cannot always be there when Mom needs me. Can you help?
A:  Yes. A Priority Senior Care, care manager can be your eyes and ears to help you manage your loved ones care. For example, we can accompany your mother to the doctor or other related appointments and communicate to you what occurred.

Q:  My loved one lives in another state, can you help me?
A:  Yes. We can provide you regular updates as well on how your loved one is doing.

Q:  What qualifications does your care management staff have?
A:  Our staff has years of experience addressing the needs of the elderly and their family caregivers, researching community resources, working with the elderly and their families and the issues that arise with these relationships, developing specialized care plans, and educating families so that they are empowered to provide the best care for their loved ones. Staff is certified by the National Association of Certified Geriatric Care Managers.
